What is Hospice?

Hospice is a patient and family centered approach to end of life care. Hospice respects the needs of the patient and their family as they face a serious illness where curative treatments are no longer successful. Services can be provided in the comfort of one’s home, assisted living facility, inpatient facility, skilled nursing facility or wherever the patient resides. When a cure is no longer possible, the focus of care shifts from managing the disease process to improving the quality of life. Hospice care treats the person, not the disease. Under the direction of a physician, and in collaboration with a team of healthcare professionals, a plan of care is developed to meet your specific needs and goals of care.

Who Pays for Hospice?

Reliance Hospice and Palliative Care is Medicare/Medi-Cal certified and accepts Medicare, Medi-Cal, private insurances, and other forms of reimbursement for services.

The Hospice Team

Hospice Physicians

Works in conjunction with your attending physician and the hospice team

Registered Nurses

Manages the coordination of services to ensure a comprehensive approach to pain and symptom management.

Hospice Aide/Homemaker

Helps with personal care such as bathing and feeding.

Social Worker

Provides emotional support to the patient and the family. Assists with financial and community resources.

Spiritual Care

Provides spiritual support to the patient and the family. Coordinates services with the community clergy members.

Hospice Volunteer

Provides a variety of volunteer services for patients, families and caregivers.

Dietitian, PT, OT, ST

Available for consultation.

Bereavement Services

Grief support is offered throughout your care as well as bereavement services to friends and loved ones.
